They found that the majority exceeded the recommended daily allowance, with two that exceeded the tolerable upper intake level. Less than half had supporting studies for their use, with the overall quality of evidence being ranked as poor.

Conclusions

Of the 34 supplement brands included in this study, 27 contained nutrients that exceeded the recommended daily allowance, while 2 exceeded the tolerable upper intake level.

High levels of antioxidants can be potentially damaging to male fertility, as some evidence has shown overuse can upset the balance of oxidation/reduction and lead to reductive stress (Henkel et al. 2019). Reductive stress is the opposite of oxidative stress, which is caused by the increase of reducing molecules instead of oxidizing molecules, and can also lead to cell damage.

The price for 30 days worth of supplement ranged from $21.76 to $159.64 USD.

Thirteen of the 34 supplement brands had supporting evidence for their use. Most studies were retrospective and 5 were RCTs. Most studies were based on abnormal semen parameters or high SDF, with treatment periods ranging from 30 days to 6 months.

The overall quality of the studies was poor, due to small group sizes, no assessment of treatment adherence, no mention of other supplements being used, and high or no description of loss of follow-up. Three of the 5 RCTs were found to have a moderate to high risk of bias due to the failure to report on method of randomization, no blinding of outcome assessors and loss of follow-up. The Tremellen et al. (2007) and Micic et al. (2019) studies were reported as having a low risk of bias.

Most studies only reported on outcomes relating to semen parameters and not live birth or pregnancy rates.

The authors note that the discrepancy between health claims and supporting evidence in the dietary supplement industry should prompt regulatory institutions to intervene, but complex regulations and unclear definitions of supplements hinder these efforts.

This review shows that the extensive range of over-the-counter male fertility supplements is not supported by good quality evidence. To make a well-informed decision, patients should be provided with information on harms and benefits of a treatment based on well designed RCTs, ideally with the ultimate end point that matters: ongoing pregnancy or live birth.
